Eligibility & Enrollment

• Open to farmers and members of cooperative societies in Karnataka.
• Minimum membership period of one month in a cooperative society required.
• Coverage available for individuals from newborns to 75 years of age.

How to Apply

Visit your nearest branch. Submit required documents (ID proof, cooperative membership details). Pay the annual premium and receive your Yeshasvini Health Card. Avail cashless treatment at network hospitals across Karnataka
